Adult Unit Admissions
Baptist Behavioral Health takes most commercial insurance providers, Medicare, Medicaid (including Alabama) and self-pay with financial assistance applications available to all.
Referrals to BHU for Adults from Other Facilities
For referrals, the following information is requested, if available:
- Copy of legal hold or voluntary paperwork
- Recent provider or nurse notes
- Triage assessment
- History and physical
- Labs and other related dagnostic results
- Medications list and MAR
Exclusionary Criteria
The following conditions do not meet Baker Act criteria or require a higher level of care than we can provide:
- Profound cognitive impairment
- Continuous oxygen
- Dementia alone
- Hemodialysis
- Continuous IV
Instructions for Referring Facilities
- Print and complete the application for voluntary admission.
- Fax the requested information in the section to 850.908.4048.
- If you have questions regarding referrals, call the house supervisor at 448.227.2105.
- For direct admissions from an outpatient office, a provider-to-provider conversation can be requested by contacting the house supervisor at 448.227.2105.
- Patients requiring medical clearance from an outpatient setting or those requiring a Baker Act may be referred to the Baptist Hospital Emergency Department or the closest available emergency department.
- Patients can be admitted by voluntary request, within or outside the state of Florida, by completing the voluntary form and sending it to the house supervisor.
- Voluntary admission forms can be requested from the house supervisor or obtained online at the Florida Department of Children and Families website.
