Stay on Top of Your Health With a Wellness Visit

Baptist Medical Group — Wellness Clinic is dedicated to navigating patients' health journeys with confidence, continuity and personalized support. We offer annual wellness visits and “Welcome to Medicare” visits for patients who see Baptist Medical Group providers, as well as care coordination for patients after discharge from the emergency room.

Annual Wellness Visits

These visits give you the opportunity to work directly with our care team to review your medical history, identify any risk factors and create a personalized prevention plan to help you stay healthy.

Your annual wellness visit is available once a year and is designed to focus on screenings and disease prevention. Please note that this visit does not include discussion of new medical concerns. If you have any new health issues, we’ll be happy to help you schedule a separate appointment with your primary care provider to address them.

Medical and family history assessment

• Medication assessment

• Referral for screening tests

Medicare Exams

• Initial preventive physical exam (IPPE): “Welcome to Medicare” is only for new Medicare patients. This must be done in the first year as a Medicare patient.
• Annual wellness visit, initial: At least one year after the “Welcome to Medicare” exam
• Annual wellness visit, subsequent: Once per year (more than one year + one day after the last wellness visit). Some plans may allow calendar year visits.

Appointment Preparation
To save time, we encourage all first-time patients to download our new-patient paperwork form. Print the form, fill in all information and bring the completed packet to your appointment. There may be additional items to process upon arrival.

What to Bring
Bring to the appointment your completed new-patient paperwork, a valid photo ID, current insurance card and a list of current prescriptions, supplements and over-the-counter medications you’re taking. Patients are always welcome to bring a significant other, family member or caregiver with them to appointments.
