  • Relevant imaging reports or medical records, if available

Family members, caregivers, or support persons are welcome to accompany you to your appointment.

Digital Registration
Baptist Medical Group offers convenient digital registration to streamline your check-in experience. Three days before your appointment, you will receive a text message from 866.716.9656 or an email from noreply@phreesia.com with a secure link to complete your registration online before arriving for your visit.

Arrival
Please arrive 15 minutes prior to your scheduled appointment time to allow sufficient time for check-in.

Missed Appointments
If you are unable to keep your appointment, please notify our office as soon as possible. We request at least 24 hours' notice so we may offer that appointment time to another patient in need of care.

Insurance and Billing

Most Insurances Accepted
We accept most major insurance plans. Please contact our office for information regarding your specific coverage and benefits.

Financial Policy
Co-payments, co-insurance, deductibles, and outstanding balances are due at the time of service. For your convenience, we accept cash, check, and major credit cards.

Patients are financially responsible for services received, regardless of insurance coverage decisions. If you have questions regarding billing or payment, our staff will be happy to assist you.

Referring to Our Practice
Order Facilitator is our preferred referral method; however, we also accept referrals by fax at 850.437.8381:
